I don't think it's a conscious thought. It's just that she has a deep aversion to her own body due to past sexual trauma. Controlling your body by fussing with plastic surgery, eating less or more, trying to appear as a man, these are all comforting things for someone who feels like nothing they do has protected them so far. She's running away from trauma by focusing really, really hard on what feels like a "project" with an end goal (like @Billy Bob Dick said, this can be a healthy response if it's not cutting up your body ... write a book, learn a skill). It's a bonus that being trans lets her control how other people view her by forcing them to refer to her as a new person.

Not to be melodramatic, but I mourn for her. Her lost beauty, femininity, sparkle in her eye. I also mourn the many instances she was victimized into this current status.
