The logistics of this whole situation are what baffle me most.
Given that she never did the physiotherapy or rehabilitation necessary to get prosthetics, and instead opted to further chip away at her stumps to the point where recovering her mobility is now neigh impossible, she surely must have live-in or daily visits from med staff.
How on earth do they allow her to rip out her hair and now her teeth without intervening or at least noticing? They should know what medication she's on, what she's eating etc, as as far as I can tell she just sits in her room all day destroying herself and not actually tending to her own daily needs.
A while back, prior to her amputation, a doc on Fig1 posted about her with suspicions that this was all self inflicted, so surely there's been at least some degree of extra scrutiny on her markedly deteriorating health?

Kelly is absolutely as ill as anyone else with a fatal disease, and I think it’s natural to feel pity for her to some degree.
The thing that makes her a cow is her need for attention and former (?) addiction to blasting her sickness on SM.

Speaking of Kelly’s internet addiction, it seems whomever is her ‘guardian’ for lack of a better word, had successfully limited her internet access until recently. And she kept right on picking even without an audience. As you say, Kelly doesn't have the characteristics, nor would she have ever been able to dance with the dystonia caused by Lesch Nyhan.

I'm not convinced her self mutilation is caused by a form of encephalitis either.
Kelly has been at this for a long time now. Years. Without timely treatment encephalitis can be fatal, even the autoimmune type.
As autoimmune encephalitis progresses it can result in permanent brain injury, coma and death.
Kelly would have been permanently disabled, comatose or dead by now.

I think in Kelly's case it's psychiatric, as much as it may horrify us to believe that there is a non-physiological cause for this behavior.

I'd imagine that Kelly is way beyond any talking therapy that would normally be used to treat non suicidal self injury (currently DBT).

The only way to stop her would be to put her in restraints 24 hours a day, which you really can't do. At least here in the States, the rules surrounding restraint usage are extremely strict. Generally the order has to be renewed by the physician every 4 hours, around the clock. I've seen other patients who self-mutilate, but not at the level of Kelly. She's really in a class all her own. There's really nothing the medical team can do, she's determined to pick herself apart. Her brain is fucked.

There was that stupid movie Million Dollar Baby, with Hillary Swank and Clint Eastwood, she was a boxer who became paralyzed and on a ventilator, has amputations because of bedsores, she is reduced to biting her tongue to try to bleed to death, and has to be put in head restraints so she won’t keep doing it. Well that’s the last straw and her trainer finally gives her a mercy shot to end her suffering. Terrible movie.
Anyways, I think gorking Kelly out on drugs is the only way to keep her alive, if we are to learn anything from that shitty movie, unless they take her tongue out as well as her arms.

I tend to believe it is a psychiatric issue, too. There was a period of time 1 or 2 years ago where Kelly seemed somewhat stable for a few months at least. I've always wondered how much of a role her identity as a twin may have affected her mentally. Her twin seemed like an ambitious type...maybe it literally ate away at her mental health that she felt she could not compete with her sister? To the extreme that she literally started chipping away at herself.

This must be really horrific from the sister's perspective. How could she even look at her twin when what'd she be seeing is a vision of herself in the most horrific circumstances most people could imagine...
