My guess is that it's a higher dose than what real boys get when they start to get voice changes. The sudden push for masculation deforms the vocal chords. Actual boys in get a gradual increase of testosterone and already have higher testosterone levels even before puberty.
Some pooners do develop normal male voices, and some pooners never get any changes. Both are rarer than the frog voice but they do happen.
