It considers not only what each person actually makes, but what they could make (imputed income), relative parenting time %, who pays for healthcare insurance, dental insurance, and child care, whether anyone is receiving state support, non-joint kids and any support obligations for them, and other things. But the minimum for someone never taking care of the kids is $50/month. Because that's totally adequate.

Addresses in particular are usually publicly recorded as a deed with the recorder and tax authority. Phones aren't required so giving the number on a public court document is voluntary.
