In all fairness, I don’t think fat fetishists really care about pussy. Disagree. I think it's just her lips that are overfilled and the rest of it is fat. She has duck lips from her overfilling and the classic pillowing over her lip line caused by this. They've found that filler remains in the body even after the usual time it takes to dissolve (3-6 months depending) so if you're refilling the same amount each time that basically builds up and ends up with an inevitable overfill hence the duck lip.
She still has heavy nasolabial creases (the ones from nose to mouth) and marionette lines (side of her lips) that are from age. If she was truly doing too much on botox or filling these would be smooth or puffed.
