Let's be real here - he probably already has. It's difficult to assess the impact of many things, and almost impossible to do it when they first happen.

Just as a recent example - with the whole cutting of USAID a lot of HIV programmes overseas have massive funding gaps. You might not care about the people who now don't get medicine - but the downstream of this is that those people are now going to be at higher risk of spreading the disease. People with HIV who are properly medicated have effectively no viral load, and for all intents and purposes can't spread the disease.

HIV doesn't stop at borders, so having areas with increasingly large HIV positive populations that are active spreaders, is going to cause the disease numbers to spike worldwide - including in the USA.
