I have no doubt that Nick has some sort of sleep disorder, but it's not because of an unavoidable disease, it's just that he's an alcoholic who stays up late to avoid his wife and kids, and that habit has ruined his internal rhythm. He is the disorder.

I doubt this 'narcolepsy' existed when he was studying/working and (tenuously) part of the real world. And if a doctor has diagnosed him with it, it's because of the symptoms rather than the cause.
