How hard is it to train AI off pictures of naked adult women, then tell it "okay now shrink everything down"? That seems like a very simple task for AI image generators to accomplish. You could also probably just hand it a medical textbook.

This is exactly what happens. An AI trained on naked adult women and clothed legal images of children will easily be able to infer what a naked child looks like. That is one of the main ideas behind modern image generators. The only way to stop it from happening is heavy censoring of the prompt inputs and image outputs.
