That would just mean that anybody could forge the president's signature on a pardon document and pass it off as real and get anyone off of Federal charges if they wanted. After all, your scenario would mean there's no cure for a fraudulent pardon and all pardons by "the president" are valid.

You can reverse a presidential pardon if the pardon isn't presidential. A staffer using autopen isn't the president dumbass.
You can make the argument that a fingerprint fulfills the function of a signature. So if a president is unable to sign, they could autopen and then stamp their thumb on the document as a way to say they are present and endorse the signature. But if your president is at a point they can't sign anything, then you should get a new president.
Someone could have a temporary injury that stops them from signing things for a while. Like if their dominant arm/wrist/hand got broken in a place where their fingers and thumb had to be immobilised for 6 weeks. But you'd probably want to ensure that everything they signed in whatever way they did, was witnessed, preferably on camera, and it was noted on the document that it was signed by autopen/finger print and who the official witnesses were.

Isn't this literally the point of the Vice President's position as well as the 25th Amendment? If the President is incapacitated -- for example, if he's undergoing surgery -- then the Vice President takes control until he's available again.
