Woodrow Wilson had a stroke that left him basically incapacitated for his second term. His wife took over and ran the Executive while everyone looked the other way. 2,480 pardons/commutations were issued while he was President, a pretty high number. I'm not sure which ones were hers but it's got to be a lot.
A bit of a moot point since so many years have passed, but yeah, whatever pardons he didn't make are invalid. Differentiating them however seems to be hard.
No one dared challenge them because of the immediate Constitutional crisis it would cause. Nobody knew what to do when a living President becomes a vegetable but stays alive so they all just accepted the wife for a few years. There wasn't a 25th Amendment to remove an incapacitated President then.

Nixon didn't choose Gerald Ford, Ford was forced on him after his sitting VP Spirew Agnew was forced to resign for corruption. Nixon resigned eight months later.
That part I actually didn't know about, but I had recalled from school way back when there having been a big controversy over Nixon having been pardoned by his own VP after the latter assumed the Presidency.
