We're currently at the "If you don't go asap, your life is going to take a turn for the worse" phase. I don't really want to see the "Sorry bud but your jaw bone is deteriorated too much, you did the best you could every day" phase.
I mean honestly, what are the chances of this happening ?

I really don't know much about teeth and mouth infections other then what I've read on here. His health is obviously terrible, but he still seems to "function".

With the way things are going what would be the next steps in his health deteriorating? Is he at the point where he could simply go to sleep and not wake up? Would there be some sort of breaking point where he'd have to be hospitalized?

As other have said, sepis, but more specifically heart failure. To simplify things exponentially, theres a huge correlation between gum disease and heart disease. The type of bacteria that thrives in periodontal disease wreaks havoc on the cardiovascular system and it just so happens that the teeth and gums are nicely placed right along several arteries and viens that are relatively close to the aorta.

A sudden cardiovascular incident could, in theory happen at any time at this point. But humans are funny things. When I was working we would have some people with teeth similar to Cobes who had lived that way for years. We had others who only recently entered the level 4 periodontal disease and when we referred them to an oral surgeon later found out that they had passed away before they could even get worked on. The human body is unpredictable
 
We would be saving his life if someone got a GoFundMe organized to pay for all the tooth extractions. Too bad there isn't a responsible third party available to make sure it actually gets done and isn't spent on boxes of Franzia White Sippendale and DoorDash.

So it's essentially the luck of the draw when it comes to how much time Cobes has left if there isn't some sort of intervention.
 
His bacterial infection has existed for such a long time without any treatment that you'd have to start him on broad spectrum antibiotics like Clindamycin and even then, the bacteria may be lodged so deeply in the roots and torn everything apart that the antibiotics won't even get to the infection. An oral surgeon should do an x-ray to identify this and do some root canals before then.
A root canal would only be an option assuming that there is physically enough root tissue left, but more importantly healthy and solid bone left in our boys jaw.

we know his teeth are obviously rotten but his gums indicate he has severe periodontal disease which causes bacteria to eat away at healthy bone. His gums are a pale pink with a slimy sheen and most alarming is how "overgrown" they are. There are several photos many pages back where it almost apears that his gums were growing over portions of his teeth (when he had them) this is known as gum hyperplasia and its one of the last stages of periodontal disease. Basically theres so much infection and inflammation the body doesnt know what to do and creates more gum tissue to try and cover up the infection which ironically makes everything worse.

Our boy is really, truly past the point of no return. He is not a canidate for any salvageable dental repair. No root canals can be done. He most assuredly lacks the bone needed to do any sort of implants or permanent dentures. He would need months and months of cleaning and bone grafts at this point and with the amount of infection he has, the bone grafts will most likely fail. It's depressingly sad because this was all so preventable even just a year or so back.
 
Even if an oral surgeon somehow managed to get him to survive defanging and heavy antibiotics while abstaining from drinking during the initial fitting of dentures, I'm of the opinion that he'd never keep up with the fitting/healing arc of that comes later.
A significant percentage of regular old folk/recovering addicts who end up getting dentures throw in the towel during the healing phrase and never go back to get them refitted because of how awful the first round of healing/fitting is.

From what I've been told/witnessed, getting all your shit yanked and a pair of dentures jammed into the stitched up and bleeding mess to mechanically form the healing tissue into shape is fucking gnarly, and the phase where you have to callous up the inside of your mouth and get used to wearing/eating/talking with them is no picnic either apparently.

I suspect that's why the whole denture adhesive market even exists, for everyone with their original set of dentures that no longer fit after they healed and the swelling went down.

It's my understanding that you're supposed to go back and get them relined/refit to your mouth after all the stitching heals/swelling goes down, so they again fit flush and stay put due to adhesion from your oral mucous alone, but a lot of people never come back and just make due buying pink goop the rest of their lives.

If you somehow got him to survive and keep up with all of the above, I'd argue he'd be an excellent candidate for fatal overdose brought on by mixing his pain RX and drinking and that'd be that. Plus at his young age I'd think they'd want to try and keep some teeth and root canal them and turn them into capped abutments to help hold it all in place and so he doesn't get early onset jaw shrinkage/end up with that flat/meth faced look you get with full dentures, and he seems to be light years beyond that point. The abutments would have to be full on implants and that jacks the price up way crazier/who knows if his immune system could even handle that anyway.

If he does get em yanked, he's gonna be old Gummy Joe from the Simpsons, we're not likely to see much of a customized fangs arc, getting dentures and learning to live with them is far more arduous than say, fixing a flat tire on a bike.

TL;DR, go brush and floss, right now.
 
Lurker with a medical perspective, as requested earlier, from working in an ER and ICU for 4 years as an RN and Paramedic/PHRN.

In regards to his medical failure points there'd be a few that are becoming pretty relevant. His teeth are obviously giga-fucked, which yeah there is the possibility of systemic infection. However there are two conditions that are grossly apparent. One his liver is kickin it, evidenced by his gut. That's not just fat boys, but ascetic. It is a pretty good indicator that his liver damage has passed the point of recovery. Honestly the way he's inflating, he's going to need a abdominal paracentesis (big needle to drain fluid), so all that fluid doesn't crush his heart and lungs (seen it more than once).

Secondly, I think he's definitely a candidate for heart failure. His extremities are also puffing up, and don't seem to be just fat. It appears to be edema (fluid buildup in extracellular space due to inadequate circulation, bc the heart is shitting the bed). Another thing that points to that is he's frequently tachypneic at rest, which admittedly could be his fried lungs, he's kinda young for significant COPD development. So my thought is that it's pulmonary congestion from said HF.

Either of those things could cause an emergency event. Cardiopulmonary collapse from compressed organs is again possible but he'd have some time to react. However the more concerning thing would be flash pulmonary edema from suspected HF can result in respiratory failure then arrest in a handful of minutes.

All speculation really, but however you cut it, it ain't looking too good toobs.

If I saw a video named "The decline of KingCobra" from an actual medical expert I'd say to myself "that goth doctor smells good that's whats up". I need medical analysis on the boy because he so fascinating. If not now we'll get lots of it once he unfortunetly passes.

I  would solo make a vid, but i fear that while covering the subject wouldnt be an issue, making it Entertaining would. Like it would probably take the form of a boring ass powerpoint with me babbling for an hour.

I've never made a vid before so I am certain it'd be pretty shit. But if people want it bad enough, I could be talked into it....
