Frankly, any normal person who has dealt with a narc on a close basis probably has as much of a grasp on them as a psychologist, especially one who hasn't. They're all the same. It's like they grow them in a vat.Or I could be completely wrong on all accounts. Who knows? It's the internet. Reading people can be quite a difficult thing.
I've never really been burned by one, unlike nearly all the rest of the cluster B psychos.
I suspect the people with really good "narcdar" who could tell what Nick was up to at the start have experience with such people.
I don't know if they prescribed him IR (anyone who would is a complete quack), but if they're prescribing him XR and he's grinding them up to snort them to get an actual high from it, he's also going to get an actual crash from it (IR sort of tapers off at the end rather than crashing you so if taken not abusively like Nick does will not disrupt sleep). Both drugs have long enough half-lives to impact sleep (Adderall = 13 hours, Provigil = 15 hours). They bioaccumulate and blood levels build up over several days.
This is sort of like how a meth freak will go on a run for days and then crash, sometimes being nearly comatose for lengthy periods of time like 24+ hours. That's also part of meth mouth because they drink lots of sugary drinks and then fall asleep with their mouth gaping open so their teeth are dry and exposed to the air and bacteria can go hog wild.
If you've ever seen someone in that state it's actually scary because they look dead and are barely breathing.
