My guess is a simple mix of SSRIs and isolation. He does not seem to have friends beyond the few Anus allows him who can be cut off at any time for any reason, making any interactions extremely uncomfortable as a rule, and that kind of isloation can straight up rot away social awareness and ability even before you consider how nervous he is of saying something that is judged poorly by Anus.
