Come to think of it, I don't read a lot of sci-fis so I might not be much help:
Jojo's Bizarre Adventure: I noticed you said you're frustrated with the state of Western comics and I feel this could be an antidote as it's about people with superpowers trying to outsmart each other.
Bloody Monday: Conspiracy thriller.
Delicious in Dungeon/Dungeon Meshi: A fantasy series with substance that manages to be a hit with RPG fans, fantasy fans and cooking fans.
Level E: Alien comes to Earth to study the planet but likes to troll people.
Steins;gate: College students accidentally invent time travel and it puts a target on their backs by a conspiracy. I can only vouch for the visual novel or the anime so I'm not entirely sure if the manga adaptation is just as good.
Psycho-Pass: You ever seen the movie Minority Report?
Fist of the North Star: Bruce Lee is dropped into a Mad Max world (not literally) and murders bad guys.
Case Closed and Kindaichi: This was mentioned earlier so I'll add my two cents. They are murder mystery detective series and the latter was clearly written by Columbo fans.
Attack on Titan: Bit of a divisive title because of the direction it went midway but I think it's a decent action romp.
Pop Team Epic: 4 panel shitpost series, no real plot.
Berserk: Dark fantasy. There's explicit content but not in a fanservice kind of way.
Yotsuba: Light hearted series about a kid going through life discovering new things. You can rest easy that this is not a lolicon series.
Chi's Sweet Home: Another light hearted series about a cat trying to find her mother but gets adopted by a family. I recommend this to anybody who likes cats.
With the Light: A drama series about a mother raising an autistic child. I feel like I have to recommend this for obvious reasons.
Detective Conan (released as Case Closed in some English-language countries for whatever reason).

Don't be fooled by the cute kids on the cover. It's a murder mystery series, and sexual content is borderline nonexistant. So if you like murder mysteries, you will like this.

There's another murder mystery manga (not by the same author) called the Kindaichi Casefiles, which tends to have more intelligent mysteries, but unfortunately its worse than Conan in other areas (most notably, it has more pervy content and characters who are closer to standard anime archetypes). I like both but there is definitely a feeling of "trade offs" when going from one to the other.
While I will not deny that Kindaichi has fanservice, I don't think the fanservice is that distracting. At least I hope not to the point it would turn away OP.
All You Need Is Kill is a good, quick sci-fi series.

Hadn't heard of this one before. It's neat so far.
In case anyone is curious, All You Need is Kill was adapted as the live-action film Edge of Tomorrow with Tom Cruise.

Started reading Mother Sarah again now that i found out it is finished in english (only the french had the luxury of having a finished, western version last time i checked), written by Otomo and illustrated by Nagayasu, and it seems like it fits the bill.

For the premise i let the manga speak for itself:
MS v01 008.webp
Under this story background the titular Sarah is looking for her children she got seperated from when she and her family were making the journey back to earth.
The manga is overall pretty grim in its storytelling and it pulls no punches when depicting violence. The artwork is as great as in Akira, the wonderfully "messy" backgrounds are a standout once again. The first volume might seem like nothing special but it picks up massively from there. Sadly this is one of the manga that had an international release pre-Anime/Manga boom so it's only avaible mirrored and with retouched onomatopoeia. At least it's not in colour like that first western Akira manga release. I feel this manga should be a lot more popular than it is in the West, not only because it's an Otomo work.
Got reminded of Appleseed by Shirow Masamume by the above post. Has fanservice but you can rest assured that all the women in it are legal. Deunan, my beloved.
My favourite mecha design in any manga, ever. Exceptional manga all around, the only reason why i don't second a recommendation is because it is not finished and never will be.
Homunculus
Easily in the top 5 of manga i've read in all my life, definitely Yamamoto's Magnum Opus. I give the tranny shit a pass because it is somewhat vital to the story and because i first read it long before The Great Trooning ™️ happened. Reread it earlier this year and it was as good as i remembered it, the artwork is out of this world.

Edit: Another manga recommendation that fits OP's request is "The World is Mine" by Hideaki Arai. Also in my all-time top 5 and one of the manga that IMO should be a lot more popular than it is. "Strange" is the only way to describe it, from the artwork to the themes to the storytelling. I absolutely love it.

My only point of critique is that the ending (or rather the entirety of the last volume) is one big ass pull and i bet my life that not even Arai himself could adequately explain what he meant by it.

I remember when Viz first started bringing manga to America, and you'd get a diverse array of titles. Shit like:

Sanctuary (political/crime thriller)
Battle Angel Alita (post-apocalyptic action)
Guyver (monster fights/horror)
Maison Ikkoku (romantic comedy)
Area 88 (drama about mercenary pilots)
Adolf (historical)

At the same time, Dark Horse was publishing:
Akira (body horror/action)
Lone Wolf and Cub (samurai adventure)
Appleseed (sci-fi/mech)
Outlanders (fanservice/sci-fi)

Nowadays it just feels like:
Battle Shonen
Isekai
Isekai
Battle Shonen
Isekai
Battle Shonen
Battle Shonen
Isekai

Sanctuary (political/crime thriller)
I think this one would fit what OP is looking for very well. It's not one of my favorites but i appreciated it for its mature themes and the stunning artwork by Ryoichi Ikegami. Had a release here before Dragonball established the tankobon format so it was this weird hybrid size between US graphic novel and tankobon, made the artwork really shine. All older manga were released in that format here, i remember Appleseed, GSC and a couple of others coming in that, too.
Dorohedoro
Still haven't finished it but will always love it for its super unique artwork. The show, same as Beastars, completely failed at adapting the uniqueness of the style.

Edit: I need to add a caveat to my previous post but can't edit anymore. Finished Mother Sarah and found the ending (the last two volumes really) to be quite lackluster and a slog to go through, i am not sure if i can overall recommend it anymore. I expected a lot more, especially after its excellent middle arc. I'd recommend reading The World is Mine a lot more now.

My Name is Shingo by Kazuo Umezu

My Name Is Shingo v03 c003 Program 3 [Consciousness] - 253 [Gantz Waitingroom].webp
An industrial factory robot arm becomes sentient and seeks its "parents," 2 children who played on its keyboard and caused it to become self-aware. Done by the absolute legend Umezu, one of the most bizarre mangakas to ever live, and who greatly influenced Junji Ito among others.
I really enjoyed this one. Never read anything by Umezu before, so I guess I should check out some of his other work.

One girl is an underage sex worker? Also isn't it fanservice? Weird choice with OPs requirements.
Yeah, definitely a weird choice. The OVA is less fan servicey overall, though I do seem to recall a dumb scene of Rally in a bra and panties and maybe a brief shot of her nipples. Compared to the manga, where it felt like Sonoda was writing it one-handed half the time. You had May, the underaged former prostitute bomb expert who takes medicine to stay young looking or tries to take medicine to NOT stay young looking (depending on the translation, I guess)
who is wet for her 30 something pedo boyfriend (she gets a couple of fairly graphic scenes in the manga), Rally our hero, who often gets naked and depantsed and threatened with rape multiple times (and seemingly outright raped offscreen once while drugged), Misty, also underaged, naked multiple times including once in bondage gear, Becky, naked at least once, a female lesbian mafiosa villain straight out of a shitty 90s Joe Eszterhas script who wants to make Rally her slave, and weird upskirts of little girls.
It's a shame Sonoda didn't have a tardwrangler to contain his pathetic goonerism, because the manga is also full of excellent car chases, cool ass gun fight scenes, realistic to the era art of Chicago, and many of the cars and guns are expertly rendered.

Since I do research on the subject (because I fucking can) I'll throw out this hidden gem.

It's called Monkey Peak. Well written and IMHO very very nasty horror (both physical and mental Mind Fuck).

When you have the time I suggest you read it.

Yea I've got decades of stuff that I have read so heh. What people read now in modern times sometimes does not compare to the writings of the masters of the past.

Which I am so fortunate to meet some of them.

Monkey Peak is amazing. The horror is okay, but its a dynamic story, one with interesting characters, enough mystery with who the monkeys were/their motives, and also enough 'fight scenes' that are less about the fight, more battles of willpower and creative ways to survive.

Excellent adventure

Boku no Pico.....

Yami no Moribito
Helter Skelter ( highly influential)
Gunnm
Kakukaku Shikajika
Nausicaa

Jojo is a series about men dressed in women's clothing striking runway poses at each other, and you have an issue with some very brief female nudity from the first series.

Get mental help. You need it.

I find the idea that Jojo of all things has loli fanservice fucking hilarious.

Resentiment is honestly great for Otaku haters. It does have lolicon but the point is to highlight how insane the culture is.

It's ironic tragedy, not humor. The point is that the level of escapism portrayed is revolting and self defeating. It's very telling that the weeaboos hate this manga for being a mirror for their sad obsessions.
Oh the otakus HATE that manga? That's a good sign lol

Girls' Last Tour and Shimeji Simulation by Tsukumizu are both pretty good. I would recommend reading the former first and if you like it reading the latter.

GLT is about two girls exploring a megastructure wasteland city in the style of BLAME!
Shimeji is kind of hard to describe it has some philisophical musings and is a yuri about reality breaking down

manga is considered low brow entertainment in its native country for a reason, get into japanese live action movies instead if you want content tailored to more mature and less degenerate interests, specially older and arthouse stuff
