I agree that seems to be part of his problem, but I don't think those are explanation enough for his theory of mind issues. If someone had normal TOM, which neither BPD nor NPD should change permanently, how would they continually think an instragram model would be impressed by a dollar? Why would they think being a paraprofessional is something you bring up when you hit on an internet model? People with BPD seem to actually be able to read people better than healthy people when in a normal state. Likewise narcissist people actually tend to be more superficially charming and successful (in the short term) with people they want something from. In fact, multiple questions on the NPI deal with this. Russ seems to actually have this backward, where he impressed his classmate he wasn't interested in, but he acts like a lonely loser to the women he actually likes. IMO, it's very likely a third factor causing ASD-like symptoms is at play, especially given its comorbidity with Moebius syndrom.

I don't think he expects them to be impressed by a dollar so much as the fact that giving anything at all should signpost, "Supreme Gentleman is Here." From that point, they should be paying attention and he can put them permanently in thrall with discussion of his high-flying legal career, litigious successes, female conquests, good looks and sexy disability.
I don't think he's on the spectrum, weird as it sounds, because his social instincts are within normalish boundaries except where he (and his humungous ego) is personally concerned. It seems like Celebrity Worship Syndrome.
