I remember reading about a murder case some years back about a guy that was fisting his girlfriend and for some reason they got into a fight during, and he ended up gutting her by basically grabbing hold of her insides and ripping them out. I could imagine it being very possible that a guy could get wrist deep and pull his chick around just with the implied possibility of something happening in that vulnerable area if she didn't comply.
