"Micropenis" is a term that likes to get thrown around a lot but it is a genuine condition. There's an array of causes, but it's all tied to prenatal development. On top of that, he's just fat. A lot of public fat can make the penis seem smaller than it is.

I wasn't being facetious when I said "he must have felt like he had no choice but to be trans." Boys with micropenises or botched circumcisions were often the test subjects for sexual reassignment.
