I have to conclude the idea that it was intentionally inflammatory. Neil wants people (sane ones at least) to give him some grumpy outrage over this tasteless scene so he can get some victim points off the "alt-right transphobe trolls" who "Hate love, and the people who love, just because of their identity at birth. The ending kinda reminds me of Batman vs Superman where Batman kills dozens of Lex's henchmen but spares Lex against all logic purely because they need him alive for a sequel.

Also Ellie letting Abby live is horrifying because Abby is an actual literal serial killer and isn't going to stop killing because she killed Joel. By not killing Abby Ellie ensured dozens of more people are going to be murdered.
