That's true, but porn production is pretty much the same way. With very few controversial exceptions, performers don't step onto the set blind. They know what they're walking into. Even those "abuse" videos are staged. A performer might not know exactly what awful things will be said or done but she still has a pretty good idea of what's coming. A vanilla mainstream sex performance doesn't just spontaneously turn into a puke/scat scene "by surprise" unless everyone's in on it. Performers don't do things they don't want to do.

The whole "woman gets pressured/drugged/tricked into performing, then the scene changes halfway through into something not discussed before hand that she doesn't want but is pressured to do" thing is largely an urban myth. If you actually see that happen during a scene, it's fake (even the dumbest abuser wouldn't release video evidence of a crime to the public). On the rare occasion someone tries it for real, they tend to get caught and exposed. The industry doesn't tolerate it; it's just not worth the hassle and bad press when there's countless other performers out there willing to do edgy stuff without being pressured.
