Not a dentist, but an autist familiar enough with dental health. A little bit of gingivitis, but it doesn't look like there's any serious indications of gum disease, so it's not that bad yet. I'd put the staining to the amount of coffee, liquor, and cannabis he consumes. The mottled color is probably a symptom of dehydration. Water refreshes the patine on teeth somewhat and keeps them from looking like what they actually are - weird bones we use for chewing. People who don't drink enough water can't produce sufficient saliva to actually coat their teeth and so they look gross. The crooked teeth are serious though and he should probably get those looked at just to make sure his jaw bones aren't being slowly pried apart by them.

HRT isn't that expensive considering the common drugs date back to the mid 20th century. The expensive parts are the physicals and lab work to ensure you aren't going to drop dead from heart failure if the spironolactone disrupts your blood potassium or suffer permanent organ damage of the kidney or liver for similar reasons. As a troon-adjacent individual, I know the cost for these is as you'd expect - anywhere from a few hundred to a few thousand dollars out of pocket (even with insurance, this kind of visit triggers your deductible so he'd still probably be forking over a comparable amount). The main concerns for digi would primarily be:

1) Alcohol. Digi is basically an alcoholic at this point and heavy drinking while taking HRT can cause liver and kidney damage. A few beers every couple of days isn't an issue usually, but I know Digi can slam back an entire fifth of hard liquor in one sitting semi-regularly.

2) Dehydration. Going back to his teeth, it's pretty obvious that Digi is chronically dehydrated (probably due to his hard drinking and weed smonking combined with his refusal to drink things that aren't loaded with sugar and carbonated). Spiro is a diuretic and its 'normal' clinical usage is in relieving fluid build-up in the body as a treatment for internal inflammation (usually the heart). Combining spiro with his normal levels of dehydration could trigger organ failure (which is a risk even in otherwise healthy individuals who consume it).
