The character isn't supposed to be "transgender" He isn't acting like a female version of himself, he is acting and dressing like a specific person, his mom. Did they even watch the movie before they made their critique? He's supposed to be fucking crazy. He doesn't have a "transgender identity".

To be fair, they took a lot of inspiration from Hall-of-Fame psycho Ed Gein who very arguably had some at least troonery adjacent issues - not just cross dressing, but he'd make and wear lady skin-meat-suits including the vaginas and breasts and mentioned wanting to be a woman.
