Tinfoil Hat Answer: Propaganda.Hospitals will treat people, and send them a HUGE bill. Why the youth of today don't know that they can go to the hospital's financial office and ask for help I do not know. Most of the time if the patient is actually poor as fuck they can get the entire bill written off, or at least set up a payment plan.
Lots of poor people misusing the hospital for every ache and pain then not paying the bill might be contributing to the high costs, since hospitals might be making up the loss by charging more.
America's poor have access to health care. Some of them aren't savvy enough to know how to get it, but anyone giving a sob story about being near death but too poor to get care like a character in a Dickens novel are usually being dramatic.(Or they have a GoFundMe they're trying to promote.)
Hospitals, insurance and pharma companies can make more money off people who don't know that there are ways to pay less. Also certain political entities (D) can use the issue to push for more gibsmedats (and thus more votes).
There is a net benefit for a lot of organizations not to make it more widely known how easy it is to reduce the price tag of your hospital bill.
Realistic Answer: People are stupid and lazy.
When I was in college I actually had people tell me that keeping my scholarship was a "waste of time" because I "could always just take out loans" like everybody else. That they would rather go into debt than just fucking studying harder and finding creative ways of keeping their GPA afloat speaks volumes about the mentality of late millennials.
I would reckon its easier to cry on FB & Twatter about why big daddy government needs to gibs you free healfcare than to actually get on the phone with the hospital's financial department and work out a way to reduce your bill.
