The scar on the leg looks like a fasciotomy scar, where an incision has been made to relieve the pressure from compartment syndrome. Part of the scar on the arm looks the same. The incisions are left open and heal by secondary intention, which is why the scars are so characteristic. You also tend to lose a lot of underlying muscle mass to debridement.

How are non-binary surgeries even medically acceptable? As I understand, if a bioman went and asked to have his dick removed because he just wanted to he'd be denied and sent to a head doctor. The exception for transwomen is made because it's thought to be a condition and the dick removal is believed to help them in some way.

The thing about troons surgeries is that they're 'approved' as part of the official medical institutions, the people with credentials and power believe legitimately believe this is relieving dysphoria and doing good to the world. It's got all the fat cats clout. Back alley clinics at least, could admit it's all for money.
Tranny butcheries are given for free in Europe because they believe it is a necessity and a cure. Nullo surgeries, people getting eyes tattoo or wanting their legs chopped off are not part of the 'official' care and they have to resort themselves to private surgeons or body mod artists to get them done. Then they made up a dubious justification that they literally have a mismatch of brain and gender identity, and so these operations were considered 'humane'.
Or the case of East Germany Heidi/Andreas Kreiger who was forcibly doped on T for Olympic, Due to her being too masculinised by hormones and being a lesbian, the people around her didn't think she's worthy to be called a woman anymore (Just like how many seem to think that FtM has more legitimacy to be considered a 'man' because they don't want to see that kind of deepend voice or facial hair to be considered to belong to a woman... which is still kind of a reductionistic worldview in a way) and so she's pressured into transitioning into FtM.
The concept of gender identity took a life of its own and stick around and now we get these idiots who think that being trans is counterculture. Medically transitioning in the western world is wholly a institutional tool used to curb people on the margin of society to fit into strict expectation of gender norms and expression. This is unlike third-world tranny where they seem to be solidly part of gay subculture there but they're never 'approved' as opposite sex by government.
