Pardon the powerlevel, but I was once obese and pre-diabetic (I had acanthosis nigricans just like our gorl; it means your pancreas is about to shut down) and as someone who got weight loss surgery, it's not a quick fix; I lost no weight for years until I just dieted and exercised like a normal person. It just makes it harder to eat most kinds of food, which is no deterrent to a fatty, and makes you rarely feel hunger - but fatties don't eat because they're hungry.
So I doubt that even if AL got surgery it would fix much. Even if she lost some weight she is going to be morbidly obese until her early death unless she changes her lifestyle.
It depends on what surgery you get. Surgeries that alter the intestines in addition to the stomach usually result in some effortless weight loss (and vitamin deficiency, so it's a trade off between two health problems). If amberlynn survived a duodenal switch she would be happy for a long time.
Regarding therapy:
Adding to all that has been said, maybe she also doesn't want to go, because it reminds her a lot of the people on 'my 600lb life' which is like a show she watches religiously. If she went, she would also have to admit to herself that she is just as bad as the people she watches to feel better about herself.
I mean, it's well established that she apparently doesn't see/realise how fucking big she is.

She does repeat the phrase "at least I'm not 600 pounds." pretty frequently, it used to be 500 but we've crossed that threshold. So yes, she does think she's better than the people on My 600 Pound Life.

Welcome to the farms! I think you're right at least on a subconscious level she knows that she's headed to Dr. Now territory and going to therapy or admitting that she needs mental health intervention makes that a reality.
