Most surgeons don't want to deal with patients of this size because there's a huge chance of them dying on the table or developing post-op infections.
Finding anesthesiologists that will operate on people that large is a challenge, too. Mechanically ventilating them is difficult (you can blow out lungs) and they require humongous doses of anesthetic which can cause a lot of issues as well.
