I'm glad I'm not the only one confused by what kidneys have to do with an orchiotomey.

But if people are saying they are a scammer because of it, there must have been some drama.

I suspect that it's nothing more than him realising that putting kidneys in there somewhere attracts more views. The archive.is link doesn't work @Smutley.

He said his kidneys were being damaged by spironolactone (archive), which is a very rare side effect, and that the doctor said he must stop spiro and that his balls must go (never mind there are other kinds of androgen blockers available in the US, pace what he said in his reply, and he could always stop playing troon)

(Fun fact: spironolactone as antiandrogen is NOT a FDA-approved usage; tranny clinics do so nevertheless because spiro is cheap as dirt and most trannies can't pay.)

Given he's survived two years I guess his kidneys are still going strong.
