And that's places that use electronic machines. My state uses optically scanned paper ballots and to my knowledge, no one has figured out how to hack a piece of paper.

You're correct, though, that optically scanned paper ballots with the paper backup kept are the current gold standard at least as far as widely deployed voting machines go.

Obviously, some kind of cryptographically sound, verifiable voting would be better in terms of security, but the problem with upping security is you invariably make the system more difficult to use, and you don't want to disenfranchise people (often old people) accidentally by making a system complex enough that it becomes error-prone to people unfamiliar with it.
