Your comment about side effects of antipsychotics is interesting because that class of medication is known to cause tardive dyskinesia, a constellation of abnormal and involuntary movements which can be severe. TD is more common in patients who have taken high doses of older "typical" antipsychotics, but other neuroleptics and even antidepressants have been known to cause it. The grimacing thing she does is reminiscent of TD, but I think the characteristic TD movements are usually more subtle than whatever the fuck she's doing. It could even be ASD; some people with autism have trouble making their facial expressions and gestures match the emotional context of what they're saying.
This could be so many things, it could be psychomotor agitation caused by mania.
