On the off chance there is an actual reason for Ralph to take Xanax, other than his addiction, he will have an anxiety disorder.
Can you imagine spending your entire life fretting about the legal consequences of your actions, on top of that - the hangover anxiety he experiences must be one of the circles of hell.
If the Vickers' want to kill Ralph, they can do it by maintaining a the steady advance of legal issues that has Ralph shitting his pants.

That’s an absolutely idiotic thing to say. Before jail, he was similarly fat to how he is now, and he was mortally fucking terrified of people finding out what he looked like. I think he had only done a couple of streams, too, because originally it was all just The Ralph Retort where he could pretend to be cool and hide what a gross fat hick he is.
Losing weight in jail is probably the very thing that lead him to feel good enough for a minute to steal IBS from Tonka and have a successful streaming thing going.
As he gained it all back, his insecurities took control again and lead to this situation where he can’t be just a chill host but retardedly made the show about himself.
