Honestly, while many of these people would deny it vehemently and call us all shitlords for saying it, I think a big part of the more esoteric body modifications is to draw attention to oneself and maybe even to make those around you feel a little uncomfortable. And in that regard, saying "Hey, wanna see my nipples" and showing somebody a jar of pickled nipples, then pulling up your shirt to show that you have no nipples would probably be a lot better at freaking people out than just showing off your lack of nipples.
