She died Oct. 25, they received the ballot on Nov 3. I don't know exactly how long it takes them to do things, but I think it's entirely possible she filled it out, mailed it in, and then died before it was received. Unless there's a more concrete example, like, say, a dude that died six months ago having a vote count, then this might be nothing
