One thing I found interesting is how relatively coherent Patty is when talking about the comic in September 2020, well after the ECT. I said in the OP that Patty's apparent decline might not be as clear cut as it appears, and that's because of examples like this, as well as his writing, which continues to be as good or better than the average 18-22 year old zoomer. In uneducated speculation: Patty's current state might be less a case of additional retardation and more sort of an emotional regression or withdrawal to a comfy state, give or take some combination of neuro and memory loss symptoms. This video seems to mildly supportive; at first very Patty's very present, then after the awful tourettic attack (feels, Patty), becomes much more childlike. Might not be anything, but I think for one reason or another Patty's a lot more intellectually intelligent than presentation might indicate.Here's another interesting video I found:

Without discounting the danger or the hard work of his parents, I do continue to see a fundamentally decent person struggling with issues beyond their control, but trying. There's no malice there that I've seen, and he seems to reciprocally love his family. On the basis of everything so far it's still wholesome, it's just sharply bittersweet. I also wanted to quote this for the reference to 'soft and comfy', it's a recurring theme, per below.In this video he says that feels repelled by the LGBTQ community being soft and comfy because he needs to be loud and violent. He only holds it in because it is morally unacceptable to harm people and break stuff and having to fix broken stuff causes him extra pain.
I must say, he seems like a wholesome figure when making art, but when he talks about the rage attacks and violence in him it can sound pretty scary. Props to his parents for trying their best to deal with all of this.

He got to a point of hating his dad so much that even a single word from him could cause Bibby to start breaking things and hurt himself. He says he has always been grossed out by his dad, even before his anger problems started and he wanted to kill himself over hearing his dad make certain sounds.
I'm inclined to think there's a big sensory element of being extremely attracted to the soft, cosy, round and cute (mentioned often, and clear in most of his OC art), also possibly with a flipside of being averse to men and masculinity (with some relation to being 'grossed out' by dad), eg this from the tumblr:13:20 He takes out his hair bands because they give him a headache. He doesn't like them, but they're the only way he knows to show people that he's not just masculine. He thinks gender stuff is dumb and he's only accepr ting that gender is a 'thing' "for you people" and he doesn't understand why he has to put on a gender performance for others.

Personally, I suspect Patty's medically induced psychosis blew out his doors of perception to some degree, that on top of an already nebulous sense of gender and sexuality, and being a citizen of the troon republic of Canada. It's possible he was specifically influenced, but I think with the transhumanism added in, and the way he speaks about both, it indicates a sense of mind and body that's fairly fractal in nature. I basically think Patty tripped so hard into gender he almost came out the other side. He posted this about a month after starting HRT, and in spoiler is what he posted immediately prior, the juxtaposition of the two kind of sums up the whole thing quite neatly.

It looks like he either didn't go through with the test or it was negative. According to the tumblr, it was scheduled in March 2020 and he hasn't mentioned it since. Archive includes all tagged as Klinefelter's.In this video he says that he's looking into being karyotyped because he thinks he might have Klinefelter syndrome. Most of the video is about having figured out how to properly shave (so he passes better as a cis lady) - but the laser clinic needs him to show up with beard hair to prove he needs the treatment.
