For him to have KS, he would have to be in at least the initial stages of AIDS. (KS is literally AIDS cancer.) Ralph got surgery in Lisbon, which would have been preceded by a full blood panel including HIV. He’s also been to the ER multiple times and would have gotten a HIV test at one of those through the years. If he were diagnosed in Richmond, the local public health authority would have chased his ass around town trying to get him on meds, as is the way in many, many locales. Could he have avoided all of this and just lived in denial? Yes.

That’s not exactly outside the realm of possibility. I’d be more shocked if they found any blood in his AIDS stream.

Ultimately it doesn’t matter which combination of neglect and disease got Ralph to look this bad. Decades of untreated alcoholism and morbid obesity would do a number on anyone, especially a low-status white Arkansas native whose only goal in life is to get his toxicology report to match Heath Ledger’s. On that day Ralph will be declared a (dead) badass and we will all feel so very owned. Sure hope it doesn’t happen too soon.
