I would also say he might not have been all the way hard in the sniff tape. He likely has boner pills and his drugs now.

Even if that was a possibility, considering the angle and distance of the photo from his penis, he would have to move a truckload of fat halfway up his torso and dig his phone halfway through his abdomen to make the picture feasible. This is excluding the fact that it's obviously not his penis.
 
I could believe Ralph is using a prosthetic, it's not uncommon in porn for men with "ugly" or small dicks to be fitted with a fake one for the scene. With all the porn work Scarlett has done it would not surprise me if that's what they're doing.
