I think my favorite video game mystery is the unicorn fountain in Ocarina of Time. Ocarina of Time was originally going to have an expansion pack which added in more dungeons and story to the game, that was going to be on the n64 disc add on thingie. But due to the disc drive thing apparently being shit it eventually ended up being canceled and turned into Majora's Mask. The only real evidence of the expansion left in the game is a cave in Zoras domain that can't be accessed because kid Link can't dive deep enough to reach it, and the water is frozen over as adult Link. The room inside the cave looks like a sorta unfinished fairy fountain with a large unicorn In the center.
 
The development cost for Gunpoint is only $30 for a version of Game Maker. The developer even had no formal background in programming and only worked as section editor for PC Gamer magazine.

It's also worth noting that a pseudo expansion pack for Ocarina of Time DID come about. In the form of Ocarina of Time's master quest, which was only released with the Gamecube port and the 3DS remake. It only redesigned the dungeons so they'd be harder similar to the second quest in the original Zelda and Zelda 2.

It apparently creeped out one voice actor so badly that he refused to record his lines and demanded that the scene be cut from the game, which it eventually was.

It's bad enough deciding to be the morality police for a game you're not a fucking writer on, but to do so after playing fucking Shillinger is hypocrisy on an insane level. There's a scene in Oz where he rapes a kid using motor oil as lube, but apparently roboticizing someone is just crossing a line.
 
--The original copies of the "Spider-Man: The Movie" video game (2002) had a cheat menu. One code, "THEGIRLNEXTDOOR," would allow the player to unlock a Mary Jane Watson skin. This cheat was removed from later printings of the game due to gamers being able to stage a girl-on-girl kiss during the game's final cutscene.

lol! I totaly remember that. Funniest part of the game.
 
There's a level in Duke Nukem 3D called "Warp Factor" that is shaped like the Starship Enterprise

You can find a secret area where you get teleported to the bridge, which is a recreation of the bridge from The Next Generation. And once you get to the captain's chair Duke says "Looks like I have the com"

Also Interceptor entertainment (The guys who made Rise of the Triad 2013) bought out 3D Realms and have access to their offices and stuff. They mentioned finding hundreds of backups to betas of games like Duke Nukem 3D and Shadow Warrior. They've been releasing some of it to the public and it's really astounding. They're planning to release much of it once they relaunch 3D Realms's website.

Saints Row 4 has a special edition that costs 1 million dollars, giving you other stuff including a trip to the fucking space.
 
From the Ace Combat series, we have the ADF-01 FALKEN. It first appeared in Ace Combat 2 as an enemy fighter flown by the ZOE Commander if and only if you shot down all the previous ZOE pilots (thereby going for the bonus ending; miss one and you'll encounter him in the next mission in which the next-ranked ZOE should appear). Ace Combat 5 marks the first time the FALKEN can be flown by the player, only after destroying specific hangars during the course of the game.

The guy who invented the Dpad (and later the Gameboy) started out as a janitor at Nintendo's manufacturing plant.

Mario's name is based on a real estate developer named Mario Segale, who owned a warehouse that Nintendo rented in Washington back in the 1980's. Legend has it they noticed a similarity between him and the sprite they used for the protagonist in Donkey Kong. So the name Mario stuck.

I heard something really cool about one of my favorite childhood games the other day. Apparently, Resident Evil was originally planned to be a 3D remake of the video game version of the famous japanese horror movie sweet home. But because of liscensing issues the game was almost abandoned, but the creators had put so much work into it already that they desperately wanted to at least try to finish it so they reworked most of it into what it is now. The reason the game was given the rather cheeesy names it got, Resident Evil for America and Biohazard for most everywhere else, is simply because the developers didn't think that it would be a very successful game so they more or less just gave it the first scary sounding names they could think of.

Also at one point in time there was a VERY impressive port of the original game being made for the gameboy. Surprisingly they actually managed to recreate most of the games content for the gameboy while keeping most of the gameplay the same. But for reasons I can't rember they abandoned it after some time.
