Metroid II: You (Samus) wiped out all the Metroids, including the very last one in the game. Well, that was your mission - so, whatever. Onto the next game, as it's revealed... UH OH - Mother Brain and her forces of evil monsters are back to take the universe! Onto Super Metroid (If we can even call it that.).

Super Metroid: You've been dispatched to eliminate Mother Brain on Zebes (skip Ceres Station, because there's no last Metroid to keep Ridley from getting his talons on.); you reach Mother Brain - she reveals her second (true) form...

YOU DIE, as nothing you have can beat her true form and there's no grown-up Metroid to save you, because you (Samus) wiped them all out! Including the one that could have saved you and the universe from Mother Brain (and her army of evil monsters)'s tyrannical rule over said universe. Have a nice afterlife, my dear Samus.

AND WITH THAT, GAME OVER.

However, by some miracle pulled outta your ass, you did beat Mother Brain in her second form without the last Metroid' s help... Well, you're gonna die in the next game! And with that, onto Metroid Fusion.

Metroid Fusion: You (Samus) are killed by the X Parasites on SR388, because, again, there's no Metroids to save you in any way, shape or form, not even as a cure for the X Parasite infecting your ass. Because, again, in Metroid II, you wiped out ALL the Metroids, including the one destined to save you in Super Metroid, as well as that same one destined to become experimental medicine in case you ever got infected with something like that of the X Parasites!

So, you beat Mother Brain (somehow...), but now it's the X Parasites that rule over the universe. The same parasites Metroids are beloved for being predators to their prey and helping to keep the universe safe from.

AND that's why we don't wipe out the Metroids, everyone. Again, have a nice afterlife, Samus.

OK, NOW IT'S GAME OVER FOR REAL.

Chrono Trigger: Lucca's theories hinge on the wrong model of causality. Going back in time to save Marle causes Chrono to be unbirthed for all eternity. The Future Always Refuses To Change.

Final Fantasy VI: the slave crown on Terra's head is untested Imperial tech. Biggs and Wedge are two underpaid grunts who forget to warn one another about not attacking the Whelk's shell, so when Terra tries to fry it and she's electrocuted the thing short-circuits and she strokes out immediately. Kefka eventually betrays Gesthal anyway and we get Clown Regime forevermore.

Space Hulk (any game, the Amiga one from EA, the boardgame, the new first person shooter): instead of winning the day you get eaten by a tyranid. Absolutely nothing in the galaxy changes, as you are utterly insignificant. This is also the ending if you win the whole campaign in any of these, btw.
 
Recettear: Capitalism fails you as your item shop fails to meet demands of your abusive fairy loan shark, you get thrown out of your house by your landlord and forced to live out your youthful days as a street-urchin, with no living relatives nor business to run you fend for yourself, you decided to take refuge in a dungeon. Mucked with dirt, blood, fecal matter and other things. you reek like the monsters you had killed by the adventurers you helped out.

The door to a room you've staying in open up, not with a warm greeting but with a fierce stab, a end of a short sword is lodge in the young girl's abdomen. The young poor shop-keeper dies a penniless individual.

Recettear was a weird ass indie game.
 
Portal 2: After escaping Wheatley's death trap, you go back to him, whereupon he asks you to jump off into a pit and kill yourself. You oblige him. (This actually prompts a line if you do this in game where he says "Wow, can't believe that actually worked." )

Bioshock: I think the most humiliating and pointless death would be dying at the hands of Sander Cohen in Fort Frolic, where Fontaine and Ryan both just wonder what happened to you.
 
The room:

you fail to solve the first puzzle
the plot never continues
all information on the null element is lost
the villain from the 3rd game ends up being killed by the null element instead of trying to sacrifice you to it
and your life just goes on as normal

Red Dead Redemption: After storming Fort Mercer with his allies and chasing a fleeing Bill Williamson into Mexico, John Marston decides to head back to the area around the fort to do some hunting. After about half an hour of collecting $1000+ of animal skin, fur, and bones, John decides to head back to Armadillo to cash out his hunting trophies. As he's getting ready to leave, he's ambushed by a mountain lion and knocked to the ground, badly injured.

Just as he's getting back up, he tries to whistle for his horse so he can get away, only for the mountain lion to attack a second time, killing him. Bill Williamson remains in Mexico, outside the grasp of the US govt., and young Jack Marston was forced to grow up without a father.
 
Monkey Island I: You die in the hands of the sword master after s particularly cringy response to one of her insults. LeChuck force marries Governor Marley and rules the Caribbean sea for all eternity. Everyone's miserable. Stan loses his business as no one can safely sail anymore. Meathook dies of a nose infection after picking it too much with his hand-hooks. Carla and Capt. Smirk actually try to rescue the governor but fail due to "internal conflicts" and die on their way to Monkey Island. Mancomb Seepgood, Estevan, Cobb and the 3 Pirate Leaders die of stomach cancer shortly after Guybrush demise. The Scumm Bar Chef loses all of his regulars and commits suicide by eating his own stew. The shopkeeper misteriosly disappears after going out to one of his strolls through Melee Island along with the fake bridge troll. A trail of blood was found at the bridge along a pair of footprints but nobody bothered to follow it. The Fettuchini Brothers split after failing to come up with an impressive act, they never speak to each other's again. Otis rots in jail and ends up serving in LeChuck's ghost ship only to get imprisoned again for drinking all the Grog. Herman Toothrot finally gets into the Giant Monkey's head and assists LeChuck's wedding but fails to recognize his daughter. He also ends up serving in LeChuck's ship but not as a ghost and dies of starvation while sailing when he realize that ghosts don't eat. The Lookout dude lives to 115 and his skeleton can still be seen on top of the highest hill in Melee Island. The cannibals are enslaved by LeChuck's after failing to recover their magic root but not much changes in their lives. Lemonhead still sucks at making idols. The three men of low moral fiber can't leave Melee Island and their fate is unknown. Finally, the Voodoo Lady forsees Guybrush's death and escapes Melee Island before the marriage. She finds a cozy hut in the coasts of what is known today as Louisiana. Her linage still lives on.

Uncharted 4 - Nate lands on his foot incorrectly after decades of making leaps of fate, irreparably damaging his knee and tendons. He is essentially crippled and is forced into retirement before he can get his hands on the treasure.

Mass Effect - Shepard contacts Space AIDS after a drunken one-night-stand with an asari and dies.

Wolfenstein: The New Order - As BJ watches Anya lead the troops away from the castle as Fergus/Wyatt drops the bomb, he begins to realize that none of this is real. He is actually in a hospital bed, dying from a stomach infection. The whole game is revealed to have been a hallucination caused from BJ's brain damage, a power fantasy borne from his underlying feelings of uselessness and inability to help with the war after being mentally crippled. In reality, the Nazis had lost and BJ was in a mental asylum in America for traumatized vets. As BJ's dream comes to an end, his heart monitor begins to flatline. As BJ dies in his dream, so too does his miserable real-life existence.
 
SWAT 4 - After mirroring under the door and spotting the last suspect of the mission, the Element leader orders a standard Open, Bang, & Clear. Stacking up in proper SWAT formation, Reynolds opens the door while Jackson throws a flashbang directly at his own feet. The whole team is blinded and the suspect proceeds to gun them all down. Lieutenant Antwiler can be heard screaming over the radio, "YOU FOOL!!!"
 
My actual experience with playing America's Army:

In training, you're given a gun at a firing range. I shot the guy next to me who was doing his firing range training to see if anything funny would happen, and I was immediately sent to prison where all I could do was pick up a teddy bear.

Then I quit and uninstalled the game because lol fuck any game with friendly fire. My character is still serving a life sentence for immediately firing at his own allies the moment they gave him a gun.

You joke but Link dying in OoT is canon so ALTTP sealing war happens

That's how all the best Zelda games happen too, so in my opinion, Link does canonically die in OoT. All the other games aren't canon and completing OoT isn't canon at all, I don't care what the timelines say, the Fallen Hero timeline is the only one worth its salt.

(Breath of the Wild probably takes place at the end of the Fallen Hero timeline too)
 
Shin Megami Tensei: Nocturne: Demi-Fiend just barely defeats Matador and triumphs over him. Bruised and beaten, he makes his way back to the Manakins in the Underpass of Ginza. Unfortunately, he immediately gets ambushed by a single Sudama and gets killed by a single critical hit from the little guy flying into his face. In his moment of glory and relief, he forgot to heal himself right after the fight with Matador.

The Lady in Black, after watching him defeat Matador and telling him to go to the Labyrinth of Amala to deliver the Candelabra, just stands there. Dumbfounded by how the one to kill Matador let himself die in such a stupid way.

Regardless, with the Demi-Fiend dead, the battle for the Candelabrum continues between the Fiends. Unfortunately, before the final fiend could bring down the Candelabrum to his master, Metatron comes down and smites him/her.

And so, the Fallen Angel's plan gets set back for another eternity or so. He wonders where the hell he went wrong picking out these fiends.

Things would continue on as per usual in the Vortex World. Isamu, Chiaki, and Hikawa still find their respective reasons and gods and make their way up the Tower of Kagatsuchi. Yuko is still used as somewhat of a magatsuhi battery for Hikawa and then promptly sacrificed by him.

Who wins and gets to change the world is really anyone's guess. Although considering Isamu gets destroyed by Chiaki if you sided with him, chances are he doesn't get to see the world of Musubi come to be. At that point it's just Chiaki and Hikawa. Knowing the latter, he'll probably beat her by tricking and outsmarting her and be able create the world of Shijima. His world of stillness.

And thus, the cycle continues on. Without the Demi-Fiend's intervention, the world becomes reborn and will eventually die out, leading the way to another conception. All of which will be overseen by the Great Will...

All the while Decarabia is still waiting for Forneus to show up.
 
Nioh: Your ship sinks in a storm, you get lost at sea

Mass Effect: Shepard reaches the final ship of the Reapers. He gets Deja Vu, he's been here before. The Reapers are so advanced, any time someone is about to eliminate them, space time is ripped apart and the person that destroys them gets sent back to the point that put them on that path, in which the Reapers unceremoniously execute them. Unfortunately, since Shepard absorbed that Prothean relic, this is completely disrupted. Shepard wakes up back on the Normandy, the instant he wakes up from touching the relic. The effects of the relic negate the Reapers from remembering this time disjunction. This then repeats endlessly, resulting in a continual time loop where neither Shepard or the Reapers realize it is happening. Shepard and the Reaper War repeats for all eternity with no resolution.

Nosferatu for the SNES: The girlfriend you're trying to rescue gets turned into a vampire, then she bites you and turns you into a vampire, and you both live happily ever after biting more people and turning them into vampires.
