The Bad Ending Game

GTA: San Andreas - After 5 years on the east coast, Carl Johnson goes back home only to get stopped by the cops and dropped into an alley. Carl tries to jack a nearby bike but as he rides out the alley, he gets ran over by car full of Ballas that gets shot at by cops. The car then blows up killing Carl beneath. The doctors can't save Carl this time, due to a shootout between Ballas and Grove Street members in the hospital. Carl then dies for good in his gurney.

GTA: Vice City - Tommy Vercetti makes his way to the deal that is ambushed by the thugs of Diaz. Tommy Vercetti fails to dodge the bullets and ends up dead alongside Victor Vance.

VTMB - You die and become a vampire only to get decapitated by the Sheriff after the Prince chooses to not give a fuck over Nines objections. You died a final death.
 
Dragon Warrior for the NES.

The hero discovered his legendary heritage, saved the princess, discovered his legendary ancestral artifacts, slays many dragons, but in the end is tempted by power and joins the dark lord. The incredible godlike power that flows into him causes his perception to extend into infinity, essentially making a moment take an eternity. The End.
 
Witcher 3, Wild Hunt: Geralt dies after being jumped by a pack of drowners while investigating an anti-dwarf arsonist. Vesemir carries the bad news to Yennefer while bitching about how he never studied. Everything basically goes on as before for some time, because realistically Geralt didn't do anything that Yennefer and Vesemir couldn't have done; or indeed that Yennefer couldn't have just done by herself. However they never go to Novigrad because Yennefer's still pissed about getting cuckqueaned, so everyone dies on Radovid's pyres.

They find Ciri uneventfully and defeat Eredin and the Wild Hunt, who are still just not spoopy enough to beat human mages despite pretending to be scary skellingtons. However, Ciri dies immediately afterwards to the White Frost...somehow, due to Yennefer not condescending to have a snowball fight with her, on the grounds that it would stain her dress.

With the only two remaining people in all the world she cared about dead, Yennefer draws herself a warm bath and opens her wrists.

Radovid Makes Redania Great Again.

Shadow of the Colossus: Wander defeats the first colossus. Realizing that his quest to revive his love is indeed possible, he swiftly makes his way to the second colossus.

Through his sword, he discovers the second colossus has weak points under its hooves. He just needed to shoot at them with his bow and arrow in order to create a path for him to climb.

He stands in front of the colossus, it stares down at him, then it lefts its front legs up into the air in an attempt to stomp on our hero. Wander carefully aims his bow, his heart racing as he stares at the underside of the creature's hoof. He takes a deep breath and fires.

He misses. Before he could get another arrow out, the world suddenly turns black.

Days pass, without Wander being able to kill the colossi, Dormin could not hold his end of their bargain. And so, Mono's body is left to rot and decompose.

Lord Amon and his followers eventually make it to the Shrine of Worship, but they find only one of the statues broken and Mono's rotting body on the altar. They could only assume Wander failed in his quest, and lament over someone so young doing something so foolish, yet well-meaning. They proceeded to scour the Forbidden Lands and eventually found and recovered the Ancient Sword. They pray to Wander's corpse before they made their way back home. At that point, they would take special measures to make sure a tragedy like this would never happen again.

ICO: As the bridge separated and began to pull the two apart, Ico watched as Yorda went farther and farther away from her. By the time he realized he could have jumped to her, it was too late. The gap between them was far too large for him to jump. He could see Yorda calling to him. She was clearly horrified over what was going to happen to them both.

Ico turned around and saw the line of idols blocking his way to freedom. If Yorda was with him, they could have moved them out of the way and they could have escaped.

But he was alone now. He couldn't get past the idols on his own, and the bridge he was on was slowly disappearing into the cliff walls. Below him was nothing but ocean and sharp rocks.

He suddenly realized that this was it. He was going to die. There was nothing he could do. All he could think about was Yorda, and what her mother was going to do to her. Despite only knowing her for a short time, and despite the fact he couldn't understand her language, he grew to really care about her and saw her as his absolute best friend. And yet he failed to save her. Perhaps if he could have reacted faster, he would have jumped the bridge and protected her from whatever would come after them, like he did numerous times when they were together. But he didn't.

Eventually, the bridge was gone, and he fell. His fate was sealed.

Yorda's mother would perform her evil ritual and take over her daughter's body so she could continue living on. Yorda would no longer be herself.

Yakuza series - After recovering from his wounds and retiring from the Tojo Clan, Kazuma Kiryu receives a phone call begging him to help sort out the chaos in the wake of his successor’s death.

His reply to this request is “Not my circus or my monkeys anymore, sort that shit out yourselves and leave my name the fuck out of it.”

He retires to Okinawa and takes up golf and sport fishing to while away his days.

Pokemon Red/Blue/Yellow/Green: Red decided to just sit at home and play Super Nintendo. Blue goes on to defeat the Elite Four and become the region's Pokemon Master. Nothing in the world changes, though it's ambiguous as to whether or not Blue ever filled out the whole Pokedex. Blue is later defeated by Ethan or Kris in Gold/Silver/Crystal anyway.

Super Mario RPG: Bowser kills Mario once and for all at the very beginning of the game. Bowser, now homeless, goes on to try and fight Exor by himself, to no avail. Princess Toadstool is forced into marriage with Booster, though nobody strong enough to conquer his tower will come to save her. Mallow will never make it back to Nimbus Land and likely continue living with Frogfucious, and Geno won't get to do anything significant. It's very unlikely that Mallow, Geno, and Toadstool will ever meet one another.

Animal Crossing: The human that arrived on a train and took out loans with Nook eventually closes themselves up in their home, never to leave again. Some animals move out, some stick around forever, worrying while eternally counting the weeks since the human had last been seen. The entire town eventually gets overgrown with weeds, and Nook will never have his loan repaid.

Oh wait, that's the case with everyone's Animal Crossing save, sooner or later.

Soulcalibur 6: The conduit loses a duel with Azwel in the desert, and fades from their wounds, thirst, and inability to draw mana from the astral fissures. Azwel decides they are too weak for the purpose of ascending mankind, presumably looking for another host for this purpose.

In the grand scheme of things, the world is still seeking the two swords out for their own ends.
 
Legend of Zelda 2 Adventure of Link: Link gets killed after falling into lava. Zelda never wakes up, no kingdom and the populous just overthrows the monarchy and a dictatorship is installed.

Chrono Cross: Kid dies after succumbing to the poison. Serge loses all hope and stays in his own world forever in guilt as Lynx takes over everything then focuses on taking over Serge's world as Serge does jack squat about it.

Karnov: Karnov dies and it just says you died.
 
The Messenger:
Ninja dies, and since he has not enough crystals to pay Quarble's death, he's gone for good. The cycle continues, and the order of the Blue Robes mourn his loss as they have to wait more 500 years for another western hero to rise.

Marvel vs Capcom:
Strider Hiryu and Wolverine teamed up to bring all of the brainwashed superheroes from Onslaught's mind control. However, Onslaught destroyed them both with giant beams of doom. Onslaught proceeeds to take over both universes.

TMNT Hyperstone heist:
Shredder undoes the mutations with the Hyperstone. He then proceeds to slay Splinter and sends the Earth to Dimension X, where he conquers the world.
 
No Man's Sky: The Atlas, after revealing the plight of itself and the universe to the Anomaly, is perplexed to witness the Anomaly walk back to its starship, get in, get out, then detonate themselves with plasma explosives.
Nothing really changes, as NMS is big on nihilism. Maybe -null- gets to hop to another galaxy, and I guess Apollo just gets a bit upset about losing a friend.
 
Skyrim:
Alduin destroys Riverwood instead of Helgen so you just get executed.
 
Fire Emblem: Thracia 776
Prince Leif's fight to liberate his homeland is going surprisingly well, when suddenly he volunteers to be separated from his friends and thrown in prison in exchange for his adopted sister who's been held hostage.
A handful of strangers sympathetic to his cause sneak in to help him escape, but the armored knights guarding the exit are just too damn strong. Leif and his newfound allies are slaughtered.
In all likelihood, Seliph probably could've liberated Jugdral without Leif, but as a result of his death, his actual sister never learns of her true heritage and is killed while fighting for the baddies she thinks are her family, leaving Thracia in shambles as neither the north nor the south have any heirs left.

Bill, Zoey, Francis, and Lois fought hard on their way to Mercy Hospital. After trudging through the sewers and fighting their way through the numerous floors, all that remained was to hold out on the rooftop till rescue arrived from the news chopper that had been called over the radio. The undead were held at bay by fire and blade and as the four rushed up a ramp to the helicopter hovering in wait, on ominous mix of a groan and a roar sounded. Turning to face the threat, old man Bill aimed his shotgun and began to spray round after round towards the charging hulk of meat rushing towards the group. One, two, three, four rounds splattered gore from the bloated arm protecting the head. Just as the final few shots were to put down the roaring abomination, a voice of evil sounded from behind him. "Here take this." Before Bill could register the thought, the shotgun in his hands was replaced by a bottle of pills, allowing the charging undead to plough into him with a suicidal plunge over the edge of the building. As Bill fell to his death, he could only watch as his friends abandoned him and flew away, almost swearing he saw a smile in Louis' face before impacting the pavement below.

Civilization IV: Abraham Lincoln and his followers are eaten by a bear in 4000 BC. The lands that would have been part of the American Empire are divided by BFFs Stalin and Charlemagne. None of this ends up mattering because Sitting Bull goes to space first 6000 years later, and everyone else just drops dead of apathy one must assume.

The Sims: I tried to cook a hotdog. 7 people died.

And perhaps my favorite. The American people's history are lost to the annuls of time, when while searching for a homeland, they were all eaten by a rampaging pack of bears. CIV V

Isaac Clarke, equipped with a military grade suit and fully upgraded rig, found himself aboard the crashed USM Valor. He had fought and 'killed' hundred of necromorphs and battled creatures the size of buildings on his journey through the USG Ishimura. While searching for the Valor's singularity drive, Isaac allowed himself a second to drift into though before being forcefully brought back to reality by a volley of alien spines piercing his flesh. As he raised his line gun to fire in retaliation he was struck by a second barrage and then a third. And so it was that Isaac, along with any hope of humanity's survival, died by being wombo combo-ed to death by a bunch of zombie babies.
 
Baldur's Gate: Shank or Carbos succeed in their mission in Candlekeep, resulting in the death of <charname>, Sarevok's victory, Irenicus and Bodhi's revenge on Elvenkind, and the eventual rise of Amelyssan the Blackhearted as the new Goddess of Murder.
