Unfortunately it's probably not that dramatic. A friend said it's probably the internal intercostal muscle and he needs to

1. Stretch out his chest
2. Strengthen and exercise his back muscles
3. Spend less time sitting in front of the computer

I find it funny that he thinks he's the picture of health when he's such a fat slob.
