How about a special mention to this fine fellow?
I like his character arc, but that design... yeesh. I get what they were going for, but the overly bright pastel woodland faerie niche was already covered by Breezies! And a wing for a tail? Why?

Steampunk is a perpetual headscratcher for me. It's what you get when you don't think design through. Every aspect of character design should have a discernible purpose. Steampunk is just random shit most of the time.
But when there's actual thought it looks good.

As far as more terrible designs go in my opinion, there is one monster from a game I finished. The Suffering has monsters that are scary and all a reflection of the island it's on, sorta like Silent Hill. Out of all the enemies though, there is one I can't help but find ridiculous: the marksman
Out of all the monsters that are scary, it's the least scary. I find the idea of having rifles attached to its back to be not as scary as monsters with machetes for limbs. Even if it wasn't too bad of a design, it falls short to other enemy designs from The Suffering.

The needle dick is such a hilariously dumb and worthless touch. It got censored and removed altogether so much that it's doesn't seem like it was worth adding to begin with.

It certainly wasn't necessary from the start. It's like someone behind creating this wanted to be edgy and go there.
I'm sure it comes down to wanting to be "unconventional". I guess just rendering a frog as-is with his usual goofy motorcycle getup wasn't enough.
