I believe their disturbing appearance is intentional, considering the plot of the episode was SpongeBob trying to find someone who would adopt the wormlings into a loving home, with every potential customer either being disgusted or having unsafe desires for them.

100% i never liked any adaptions of jojo and the art is one reason why. when you try to draw odd and strange things it always comes off as lazy after the first impressions goes away. granted, the designs were different for a manga that came out in the 80s but it gets old quick and just become shitty

Honestly I found both faces to be awful, like it's probably just me but the original Spidey PS4 face looks like a goblin man while this one is bad for being uncanny valley territory
