When you "nod" or "shake" your head, do you mean yes or no?

I don't know if it's a regional thing, a degradation of the English language thing, a people using English as a second language thing, or what, but in recent years I have heard a lot of people lately say "Shake their head yes" and "Nod their head no." I feel like I'm taking crazy pills because as far as I can recall, it has always been nod for yes and shake for no. For reference, nodding is bobbing the head up and down and shaking is going side to side.

I heard in Brazil you shake your head sideways like a broken spring to say: "yes" or "ok". Nodding is also used, but not as often.

Generally to the person receiving a nod from me it means, yes i'm here, i'm sentient, i may or may not actually hear what you are saying but i'm acknowledging your existence in a conscious and active way.
