All I can think of is the novel The Outsiders… it’s a pretty common book for American middle schoolers to read. Ponyboy is the name of one of the characters. Not sure why Lucas is using it here, though. Maybe yet another attempt to appear intelligent by referencing something he doesn’t understand?
Several reviews did say there is drug use going on a Mallon Place and the staff turns a blind eye. Also keep in mind residents who are able can leave the facility for the whole day. They may not be able to get drunk/high while in the main building but there really is nothing to stop them from doing it while they are out.

Diabetics have all sorts of skin problem. Blood and therefore oxygen aren’t properly received in the smallest blood vessels in the tissues. That’s why they need to check their feet so often, because they go numb, they’ll step on something, and then necrosis sets in because that part of the foot has been essentially dead for a while anyway. I wouldn’t be surprised if his legs looked like an absolute mess.
He's making a reference to an 80s movie in an attempt to be "hip" , I'm guessing? Like he's "deep" and reads a lot.
"Hypocrite" is one of those terms that Lucas has decided is self-reflexive, along with "narcissist". His personal application of the word suggests that if you call him a narcissist, you necessarily are one yourself, and the same with a hypocrite.
