Pretty sure the primary issue with inspections on Groverhaus was that he had put outlets like every 9 inches on his kitchen ceiling, I guess to soothe some sort of power-related traumatic childhood experience and an electrical inspector told him that wasn't going to fly so he threatened to get his own certification and sign off on his own shit. I'm not sure that he ever did though.
I don't recall any of the work ever being like dangerously incompetent, it was just hilarious and inept and he put zero effort into thoughtful design or rational usage of the living spaces for the build, and he was in way over his head. Like at one point he installed a window at the foot of some stairs so if you were to fall down the stairs just right your ass would go right through.

Oh yeah, one more anecdote from that era; when he was describing his build plans he explained that a regular addition would only add a few square feet whereas the entire second floor he was adding would put him into a new category of real estate comps and would instantly double his home's value once complete. It was just completely delusional.
