In the scenes where Jazz talks about and then meets TS Madison, he seems disturbed. In a much deeper way than when he deals with regular haters. The facial expression for, "What do you mean, she said I was crazy?" It wasn't just offended. There was suspicion, but with a hint of real curiosity. So if all these grown men with penises are still happy, brave and stunning trans women, why did Jazz need to have the hormones and surgery when he was still a child?
Jazz and Jeanette met some sad autogynephilic old men in wigs over the years to put the fear of puberty into him. But the idea of other "trans" paths haven't been part of his family narrative. He didn't get to meet other trans "women" who liked sex and kept their penis.
I think the idea that TS Madison shared about how lots of "girls" go crazy after surgery is planted in Jazz's brain, and will continue to gnaw at him. Plus working at the surgeon's, he'll inevitably meet non-binary weirdos who get mixed surgical options. Or just top surgery, while leaving genitals alone. The scene with the FTM also looked like his brain was working overtime. Like it was dawning on him that top surgery to remove breasts is a thing, and he might have something to think about there-- except he's been traumatized enough by surgical procedures already, so maybe let's not think too hard about it. Or maybe he was thinking about what the FTM said about waiting until bottom surgery options are more advanced-- *that* probably brought up some feels.
