There are regions of the body where it is hard for the drugs to "reach" because of blood flow or anatomical issues, and the epididymis is one such region. I would imagine that in addition to not moving around that his fat keeps his testicles in a stable position much more of the day than an average man's. of course I don't know for sure because I am not a morbidly obese man, for all I know it could be worse.
