They exist, the serious ones are either very private, very non-English speaking, or both. Outside of Central America, you're most likely to find them among the Danza Azteca performance groups (hope you speak Spanish or modern Nahuatl, and some of them may be closed based on ethnicity/race). They deal with the illegality of major religious rites the same way other religions with banned or outdated practices or inaccessible required holy sites do (not being able to give burnt offerings at the Temple Mount doesn't stop Jews from existing, for example) -- practice the ceremonies that won't get them chucked in jail, and develop ritual substitutes, with sufficient justification to make the situation palatable. Or do illegal stuff and try not to get caught, like a lot of practicioners of Santeria and Voudou have done/still do. (The only groups with any form of Mesoamerican religious relation I know of that have been definitively documented to do this are the Santa Muerte cultists, with their bizarre syncretism of Catholicism, Mictlancihuatl worship, and focus on criminality. Do not look up photos of their religious altars/sacrificial sites if you don't have a serious tolerance for gore -- some are purely symbolic. Some are very much so not. Very fucked up shit.)

Autosacrifice (small blood offerings by drawing your own blood), traditional incense (mainly copal, sometimes tobacco), traditional food offerings (amaranth dough baked treats, pulque, octli, or other traditional liqueurs, chile peppers, and yes, high-purity dark chocolate or cacao beans are traditional), symbolic burnt offerings involving amatl papercrafts, various shells, feathers, artwork, and, in some circles, animal sacrifice (quail or chickens -- quail as human sacrifice substitute goes back 500+ years, before Cortez). Peyote wasn't/isn't common and AFAIK was/is mostly practiced in the present-day Mixteca communities in Oaxaca, or the desert to the north among the smaller nations that used to be collectively referred to as the Chichimeca. Ceremonial drunkenness is the more common traditional form of ritual intoxication in Aztec/Nahua and Mayan religion. There are a few other things out there, like ololiuhqui (a type of psychotropic morning glory, nasty disassociative from what I've read), but you really don't see that getting used outside of deep rural villages where it grows natively.

I haven't really heard of anyone who's performed any of the more extreme forms of autosacrifice, like the thorns through tongue ritual or the like, but you can find people with traditionally-religious tattoos or piercings on occasion. There's some justification for this one -- if you read through the early 15th-16th century sources, those kinds of offerings were by and large performed only by the professional priesthood or aristocracy. There are occasionally rumors of some of this kind of stuff still being done way out in the boonies in Mexico, but never seen it substantiated by any reliable ethnographers.

If you want to read more on modern survivals/revivals in Mexico, I can try to dig up a couple books and papers I've forgotten the titles of later. Otherwise, off the top of my head, "Bonfires of Culture" is a good academic exploration of attempts to preserve Aztec religion in the first few decades after the Conquest I can recommend trying to find a copy to borrow, if you're also interested in how the Aztecs themselves historically tried to deal with the questions you had. Or if you want to know more specifically about traditional offerings, I can give the title of a couple studies specifically on excavations at the Templo Mayor that document all the cool shit they've found in its buried offering caches. Minor PL, but my private library on this topic is better than most US universities' holdings. :smug:

My Mesoamerican anthropology autism had a fucking aneurysm reading that. They do but it's mostly the New Agers and it's pretty basic stuff. Like having a Buddah head on your desk.
It's like strip mall religion vs. orthodoxy. They like the parts that feel good, which is what draws them to new age crap in general, but the more complex stuff is hard and uncomfortable. It's a lot more difficult to learn, accept, and internalize things like the core concepts of Zen Buddhism and Taoism than it is to give lip service while maintaining your self-perception and protecting your ego.
As an example, it's really hard to accept personal failings, mistakes, etc. (as encouraged by many deeper spiritual practices). This is especially true if you want forgiveness and can't get it; if other people get involved (look at modern cancel culture); or if you have a significant part of your self-identity tied up in being oppressed in some way.
It's much easier to just replace the phrase "Fuck you" with "Namaste" and chug a green drink before going home to enjoy your rose quartz dildo.

There's another reason, too, for the name -- "flower" (xochitl) is a poetic metaphor and euphemism for blood (eztli), and by extension, sacrifice. Less commonly, serpents (coatl) can also be a metaphor for blood and sacrifice. And both hummingbirds (huitzil) and butterflies (papalotl) were symbolic of the souls of slain warriors who had gone to the House of the Sun after death in battle. (And hummingbirds in particular bring the sacrifice/blood subject right back in again -- their beaks were symbolic of agave thorns, which were the original needle you would prick yourself with to draw blood for autosacrifice.) Adds a whole new layer of meaning to those alternate sacrifices of Ce Acatl Topiltzin's, doesn't it?

Aaaaalso, Ce Acatl Topiltzin is one of the other names of the god Quetzalcoatl, the Precious Feathered Serpent. "Ce Acatl" (One Reed) is his daysign name* (traditionally, your birth name was the day you were blessed shortly after birth, it could be your actual birthday, or within a few days afterwards, your parents would choose the most auspicious one to lock in your fortune), while Topiltzin references his office as high priest. Unsurprisingly, you usually see him called by this name in contexts where Quetzalcoatl is appearing in his role as the archetypical high priest, rather than as the lord of the winds (Ehecatl) or some of his other guises.

*Well, it's one of his daysign names -- as usual, myths are messy. It's the daysign name from one major folklore line, which conflates him with a quasi-mythical/quasi-historical priest-king (and is the same story that sets up the prophecy of his future return that may have been on Motecuhzoma's mind when Cortez arrived (and which is its whole own spergfest I could ramble about if people want more, but tl;dr, the Aztec emperor probably wasn't immediately accepting the Spaniard as a god, but playing a very subtle Machiavellian game when he presented the weird foreigner with Quetzalcoatl's ritual garb and mask... he may have been setting Cortez up to be sacrificed, depending on how he reacted to the accoutrements)). His daysign name from the more purely supernatural, cosmic myths is Nine Wind. (And the number nine is a magic number -- the underworld, Mictlan, had nine layers, and the upper world had either nine or thirteen, depending on which codex you read.)

*Bonus Autism -- Ce Acatl being a name of Quetzalcoatl gets an additional wrinkle when you recall that the actual day One Reed is the start of Toxcatl, the most important festival of Tezcatlipoca -- Quetzalcoatl's brother, partner in creating much of the world, and frequent rival. The gods in this pantheon are all intertwined like a fractal or a spider's web -- wherever you find one's thread, you'll soon have the rest of the cast and crew if you keep following it.

Is there any reliable info on how many of those hardcore Santa Muerete followers there are? I'm aware of official numbers, but to me wide majority of those people seem like regular people who are just looking for alternatives to Catholocism.
