I was interested so I did some light googling and dang you're 100% right. Flat closure is apparently a new option for mastectomy patients that's catching on because it offers a good cosmetic result without the need for implants or follow-up. It's basically the same thing as 'top surgery ' but it looks a million times better:

One thing I notice in mastectomy vs top surgery is that almost all top surgery scars are that straight or c-shaped underboob thing, while mastectomies have a wide range of incision lines. It's almost like- gasp! - different breast shapes work better with different techniques! Who woulda thunk it?
