It's been said before but Phil most likely has multiple mental disorders ranging from narcissism to sociopathy. This isn't even including the health issues he has that he won't actually fix.

He will never change because changing his lifestyle will cost money that can go into his addictions and he doesn't think he has problems.

Is there any theories on what was in that cocktail he took in high school?

The most obvious problem is that paypal.me makes it very clear that your personal information is shared, which will make all but the most dedicated of whales shy away from tipping. Next, there's the problem of Phil having to manually play tip animations. This means that super chats and memberships no longer get animations, which may discourage dentheads from renewing their memberships or doing super chats. There is no reason for someone to do a "troll" contribution when neither the name nor the message appear in the video now. Finally, tippers now need to make their personal information public in order to tip, because, as @Little Bill pointed out, you can easily search for paypal.me users and going to their page shows their full name.

I believe the superchats & memberships still work via Streamelements, at least they did directly after the SE 404. The tip itself doesn't go through StreamElements, it goes through PayPal. He probably used a standard PayPal tip instead of paypal.me.
