Basically as I said. Guy tried to kill himself with a shotgun, slipped and blew his face and jaw off.

Long story is his name was James Vance. He and his buddy were metalheads and were heavily into Judas Priest. They took a shotgun and tried to kill themselves. James's friend went first and it worked. James went second, the barrel was slick with his friend's blood and brain matter so it slipped and blew his face off instead. This is part of what then led to Judas Priest and other heavy metal bands being taken to court because of the song "Better by You, Better than Me" which the defense claimed had subliminal messages telling people to kill themselves.

James then went and died due to an overdose three years later. He was also the inspiration for the character of Arseface in the comic book "Preacher".
