I could be wrong but Brad Pitt and the girl are definitely his imagination, I think the members of his fight club and even the house they are all living in is all imaginary too. Again I could be wrong but that's how I took it, and there's lots of little clues that took me watching it a thousand times to notice. Its been a few years since I've seen it so I can't remember all of them off hand but it does make a lot of sense. Either way its a good movie.

Amholes are indeed horrifying, as are rotdogs, but the fact that the donor site for the rotdog looks like it has been torn off or badly burned is even worse than the genital surgery itself, especially because whatever mess troons and pooners have made of their genitals is generally concealed under clothing, but the destroyed limbs and torsos of pooners are often in plain sight for everybody to see and the resulting limbs are permanently crippled with both nerve and muscle damage.

I cannot imagine the degree of mental illness that causes pooners to think that ripping off half of the soft tissue of their forearms or thighs is a good idea for a useless appendage on their crotch as well as the moral laxity of the butchers that perform said procedures.

The book makes it pretty clear that Tyler is the Narrator's only delusion, i hear the "Yeah, Marla/everyone else is another delusion" theory mostly from people that have only watched the film.
